The cheapest verified 2026 medical scheme plan open to the public is Momentum Ingwe, from R645 a month for one adult in its lowest income band. The cheapest fixed-price options are Fedhealth flexiFED Savvy at R1 155 and Bonitas BonCore at R1 275, both hospital plans that leave day-to-day costs to you.

Every low price comes with a catch, usually a hospital or GP network, a co-payment or an income band. Some products that look cheaper are not medical aid at all but health insurance, which works differently.

Below are the lowest verified prices from open schemes, by plan type, with what you give up for each. Prices are main-member monthly contributions for 2026 and were checked on 29 September 2026.

Cheap for whom: income bands and flat prices

An income-banded plan is cheapest only for lower earners. On Momentum Ingwe the top income band pays R4 546 for one adult, more than the R2 029 that Momentum Evolve costs for one adult at any income. Discovery KeyCare Start Regional runs from R1 278 to R3 430 by band, and Bonitas BonCap from R1 730 to R4 177.

Every registered plan must pay Prescribed Minimum Benefits (PMBs) in full, without a co-payment. Some of the cheapest health products in South Africa are insurance, not medical schemes. They pay stated benefits, and the PMB rules that bind schemes do not apply to them.

Cheap plans you can only join if you qualify

Several restricted schemes have low entry prices, but you can join only through the employer group, industry or profession they serve. Prices for public-service and local-government schemes are before any employer subsidy.

How to pick the cheapest plan that still works for you

  1. Find your price at your income band and household size, not the starting price.
  2. Check that a network hospital and GP are near your home or work.
  3. Add the co-payments you might face in a normal year.
  4. Ask about waiting periods. A scheme may impose up to three months general and up to 12 months condition-specific waiting periods if you have had no cover for 90 days or more. See the waiting periods guide.
  5. If you are 35 or older with no earlier cover, ask about the late-joiner penalty.

Is cheap medical aid worth it?

A registered scheme plan still pays PMBs in full and protects you from a large hospital bill. You give up hospital and doctor choice and most day-to-day cover. For a healthy person mainly worried about a big bill, it is often worth it.

Why are some health products far cheaper than medical aid?

They are usually insurance, which pays stated benefits rather than the actual bill and is not bound by the PMB rules that apply to medical schemes. Some cover day-to-day costs only and have no hospital cover. Ask whether a product is a registered medical scheme plan.
