There is no single best medical aid in South Africa. The best one is the plan that matches your household, your health and your budget, and it can come from Discovery, Bonitas, Momentum, Bestmed, Medihelp, Fedhealth, Medshield or a smaller open scheme.

This guide does not rank schemes. It matches verified 2026 plans to five common needs: a family, hospital cover only, chronic conditions, young and healthy members, and pensioners. Every price is a main-member monthly contribution taken from the scheme's own published rates and checked on 29 September 2026, with the scheme and plan named beside it.

Restricted schemes, such as GEMS, Profmed and Polmed, can be excellent, but only if you qualify through an employer or profession. This guide covers the open schemes that anyone can join.

Open schemes at a glance

The Council for Medical Schemes (CMS) regulated 71 schemes in its 2024/25 report, 16 open and 55 restricted, covering more than 9.1 million beneficiaries. Reports on the "top" open schemes vary. One 2026 analysis by Alexforbes looked at Momentum, Fedhealth, Bonitas, Medihelp, Discovery and Bestmed. Size is not quality, so this table shows range and warning signs instead.

Best for a family

Families should compare the child rate, the cap on how many children you pay for, and the day-to-day limit, not only the main-member price. Discovery also launched the Smart Saver series for young families on 1 January 2026 (Classic Smart Saver at R3 350 and Essential Smart Saver at R2 750 for the main member).

Best for hospital cover only

If you are healthy and mainly want protection from a large hospital bill, a hospital plan costs far less than a comprehensive plan. These are verified hospital plans from open schemes at the lower end of the price range.

Best for chronic conditions

Every registered plan must pay for the Prescribed Minimum Benefit (PMB) chronic conditions in full. The CMS lists 26 chronic conditions on its PMB page. What differs is how many conditions beyond that list a plan covers, and whether you must use a formulary or a network. KeyHealth publishes a clear ladder, and Momentum Summit and Sizwe Hosmed Titanium Executive cover the most conditions.

Best for young and healthy members

Young, healthy members use little day-to-day care, so a low-cost hospital or network plan usually gives the best value. Joining before 35 also avoids the late-joiner penalty.

Best for pensioners

We have no verified pensioner-specific plan data, so we do not name a best plan. What we can say is what matters more with age.

  • Price does not rise with age. Contributions must be set by income and/or the number of dependants, not by age or state of health.
  • Look at hospital limits and chronic breadth. For example, KeyHealth Platinum covers 55 chronic conditions with unlimited oncology and prosthesis benefits, and Momentum Summit covers 62 conditions.
  • Ask about the late-joiner penalty. If you are 35 or older and have no earlier cover, the scheme may add up to 75% to your own share of the contribution.
  • Claim the tax credit. People aged 65 or older can claim an additional credit of 33.3% of scheme fees above three times the monthly credit, plus 33.3% of qualifying out-of-pocket medical expenses.
  • Check your old employer. Sasolmed, Anglo Medical Scheme and Netcare Medical Scheme accept retirees or pensioners from their employers.

Check the scheme, not only the plan

A good plan at a shaky scheme is a poor choice. The statutory minimum solvency ratio is 25%.

  • CompCare reported 21.83% for 2024 and is under a CMS-approved recovery plan.
  • Sizwe Hosmed has been under provisional curatorship since 4 September 2025. It reported solvency of 40% at the end of August 2026 and expects curatorship to end in September 2027.
  • Suremed has a merger planned for 1 January 2027.
  • Bonitas apologised on 27 July 2026 for service disruption after its administrator change.
  • Medihelp reported solvency back above 25% at the end of 2025.

Health insurance is not medical aid

Affinity Health, Dis-Chem Health, Oneplan, Sanlam and Old Mutual sell health insurance, accident cover or gap cover, not medical scheme plans. They pay stated benefits, are not bound by the PMB rules and complaints about them go to the National Financial Ombud, not the CMS. Gap cover works only alongside a scheme. There is no single best. Discovery, Bonitas, Momentum, Bestmed, Medihelp, Fedhealth and Medshield all publish 2026 plans that suit different needs. Pick by the plan type you need, the network, the price at your income and dependants, and the scheme's solvency.

What are the top 5 medical aid schemes in South Africa?

By size, Discovery is the largest open scheme and Bonitas describes itself as the second largest. Momentum calls itself one of the top three and Bestmed the fourth largest in 2024. GEMS is the largest restricted scheme. Size does not show which plan suits you, so compare plans instead.

How many medical aid schemes are there in South Africa?

Is Discovery better than Bonitas?

Neither is better in general. Discovery announced a 7.2% weighted increase for 2026, deferred to April, and Bonitas 8.8%. Both have income-based network plans (KeyCare and BonCap) and hospital plans. Compare the specific plans you would buy, and note that Bonitas moved administrator in June 2026.

What is the best medical aid for pregnancy?

Bestmed Beat3 advertises generous maternity benefits and Medihelp MedVital Elect and Sizwe Hosmed Access Core include maternity cover. Waiting periods of up to three months general and 12 months condition-specific can apply, so join before you fall pregnant. A baby born during membership can be added without a waiting period.

What is the best medical insurance in South Africa?

Insurance and medical aid are different products. Medical aid is a registered scheme that must pay PMBs. Insurance such as Affinity Health, Oneplan or Dis-Chem Health pays stated benefits. Decide first which you need, then compare like with like.
